Click to request a refill for one or more of your medications.  WE REQUIRE A ONE WEEK NOTICE FOR ALL MEDICATION REFILLS.  If you are submitting a form for a refill that is needed sooner than one week from today, please note that there may be a delay.

If you are experiencing chest pain, shortness of breath, severe abdominal pain or headache, facial droop or slurring of speech, acute changes in mental capacity, acute weakness or numbness in any body part, or have experienced a major trauma: PLEASE DIAL 911 IMMEDIATELY.

Help keep our patients, staff, and doctors safe! If you are experiencing fever, cough, congestion, loss of smell/taste, or have been exposed to COVID-19, please call us to schedule.  We are willing to see patients with respiratory symptoms in the office on a case-by-case basis. 


Need to cancel or reschedule? 

You can cancel or reschedule your appointment electronically by going into your confirmation email, scrolling to the bottom, and clicking on the green button that says "Change/Cancel Appointment."
